Turn-Key Health, an Advanced Illness Management (AIM) company serving healthcare payers and at-risk providers focusing on senior populations, announces its participation in the 3rd Annual ACO Population Health Management Summit, part of the Care Coordination & Technology Conference and Exhibition, Jan. 25-26, 2017, Sheraton Atlanta Hotel, Atlanta, Georgia. John Halsey, vice president, Business Development, Turn-Key Health, will participate in the session “Build Upon Provider and Community Relationships to Promote Population Health and Clinical Integration,” Jan. 26, 2017, 8:15 a.m. – 9:00 a.m.

”The biggest challenges that ACOs face when addressing population health management is to go beyond analytics and scale care management resources,” says Halsey. “ACOs can effectively foster relationships within the local community by providing support to smaller practices, building relationships with influential community members or engaging with payers and providers at the local level. Our solution, Palliative Illness ManagementTM (PIMTM), deploys local, in-home palliative care teams – specially trained social workers, nurses and other clinicians who may be under-utilized – to identify and engage health plan members with serious illness. The model uses sophisticated, predictive analytics to identify individuals earlier in the disease trajectory, successfully integrating population health management into AIM programs.”

Using a palliative care population as a case study, the session will explore strategies for partnering with local clinicians that have specialized expertise in providing services that are valued by members and caregivers: symptom management, nutritional counseling, health system navigation, coordination with physicians, access to care and spiritual support.

The goal is to promote supportive environments during care transitions, augment centralized care management resources, and streamline access to care at the local level and in support of the medical home and primary care physician.

“We are sharing our innovative approach with ACO executives nationwide, and exploring the best ways to increase care coordination, avoid unnecessary interventions or hospital readmissions and improve member and caregiver quality of life,” Halsey adds. “Research has shown that providing palliative care earlier in the care process leads to better health outcomes, higher satisfaction with care and reduced financial burdens."
