Jeremy Martin

Jeremy Martin serves as a Senior Advisor to Martin Ventures.
In this role, he assists with the development of forward-thinking projects focused on delivering healthcare services in new ways, with special emphasis on post-hospital and community-based care models.
In addition to his role at Martin Ventures, Jeremy is President/CEO of Heritage Management Services, a company that brings healthcare management services to a family of companies that provide patient-centered Senior, Post-Acute and Home-Based care.
These services are available in a variety of settings including skilled nursing and rehabilitation facilities, assisted living facilities, outpatient clinics, and the patients’ own homes.
In addition to his time at Heritage, Jeremy has held leadership positions in several other companies focused on the delivery of healthcare services across the post-acute and community-based care continuum.
